  • Please inform us if you have received Physical Therapy elsewhere
  • Physician/Dentist Prescription may be necessary for insurance reimbursement but most insurances will pay for Physical Therapy for the first 30 days up to 10 visits. (Direct Access to Physical Therapy)
  • Please refrain from using fragrances, perfumes and aftershaves due to patient’s and staff chemical sensitivities.
  • We appreciate 24 hour notice of cancellation and reserve the right to bill the patient a $50.00 fee for missed appointments.
  • Patients are responsible for payment that insurance does not cover
  • This office reserves the right to discharge patients after three missed appointments without notification

What To Bring With You To Physical Therapy

  • Your prescription ⁄ doctor referral
  • Insurance information
  • Government issued ID
  • Any written test results: MRI, Cat Scans, X-ray, EMG, etc.
  • Please wear comfortable non-restrictive clothes that will allow us to fully evaluate the injured area and bring sneakers for back and lower body injuries
  • Your completed Patient Health Questionnaire, Patient Information Sheet, Patient Responsibility Form and HIPAA Form; you can print these forms below
  • Please arrive 20 minutes early for your appointment to allow for time to check in and finish paperwork
