- Please inform us if you have received Physical Therapy elsewhere
- Physician/Dentist Prescription may be necessary for insurance reimbursement but most insurances will pay for Physical Therapy for the first 30 days up to 10 visits. (Direct Access to Physical Therapy)
- Please refrain from using fragrances, perfumes and aftershaves due to patient’s and staff chemical sensitivities.
- We appreciate 24 hour notice of cancellation and reserve the right to bill the patient a $50.00 fee for missed appointments.
- Patients are responsible for payment that insurance does not cover
- This office reserves the right to discharge patients after three missed appointments without notification

- Please wear comfortable non-restrictive clothes that will allow us to fully evaluate the injured area and bring sneakers for back and lower body injuries

- Please arrive 20 minutes early for your appointment to allow for time to check in and finish paperwork
